Position Title: Clinical Aged Care Assessor

We are seeking an experienced Clinical Aged Care Assessor - Aged Care Assessment Services to join our team.

* Position Location: Sunshine Coast
* Terms of Employment: Full Time Fixed Term until 30th June 2029
* Salary: SCHADS Award Level 5

We specialise in assisting older adults, individuals with disabilities, those experiencing mental health challenges, and people at risk of and experiencing homelessness.

We are recruiting to deliver the Aged Care Service (ACAS). To deliver this important service, we're recruiting a range of team members across various roles and locations. ABOUT THE ROLE

The purpose of this role is to undertake holistic personal Comprehensive Home Support assessments for My Aged Care clients. The Comprehensive Home Support assessments are for people who may need entry-level support to help them stay at home. Footprints Community aged care assessors incorporate the Aged Care Assessment Quality framework to deliver high quality assessments, with a focus on client experience. The aim is to work with clients to develop support plans to achieve wellness and healthy ageing goals through linking and referral to the client's chosen support services through the My Aged Care Gateway.

KEY ROLE ACCOUNTABILITIES

* Complete Comprehensive Home Support Assessments using the Integrated Assessment Tool (and other validated tools when required) in line with program guidelines, timeframes and quality requirements using the standardised Aged Care Assessment Quality Framework.
* Complete the required volume of aged care assessments as per contractual and reporting requirements.
* In completing the Aged Care Assessment, follow the principles in the My Aged Care Assessment Manual.
* Conduct reviews for re-assessment as required.
* Take a Wellness and Reablement approach to Aged Care Assessments.
* Facilitate clients' progress towards achieving increased stability and improved quality of life by assisting aged clients, according to individual support/service plans, to access relevant commonwealth aged care services and state-based health and community support services, leisure and recreational activities, medical and allied health services.
* Working remotely within a region where you will be required to travel long distances between clients.

ABOUT YOU

* Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker with eligibility for AASW membership.
* Demonstrated knowledge and/or experience in working with people who are ageing and their families/carers, people experiencing mental illness, people with disabilities, people who are homeless or residing in marginalized housing and who may be at risk of homelessness, and people who are financially and socially disadvantaged.
* Demonstrated understanding/experience of the provision of community-based support services to the aged which reflect a wellness approach with a focus on reablement for clients, including assessment, care planning, monitoring, and evaluation processes.
* Demonstrated knowledge and experience in community networking and working collaboratively with other service providers to plan and provide consistent, responsive, and appropriate client focused services.
* Local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region.
* Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite).
* Previous experience performing in a KPI environment to ensure good quality client outcomes.

You are required to have a current Queensland Class C Open drivers license and NDIS workers screening check or current Yellow Card (or willingness to obtain) and provide evidence of annual flu vaccination and up-to-date COVID-19 vaccinations, in accordance with Department of Health and Aged Care.
