The Opportunity
As a Care Partner (or Clinical Care Partner), you’ll work alongside clients, carers and a multidisciplinary team to deliver person-centred care that supports independence, health and quality of life. You’ll coordinate and review tailored care plans across aged care and disability programs—making every interaction count.
You will:
- Complete holistic assessments and co-design care plans aligned to client goals and clinical recommendations.
- Coordinate and monitor services under CHSP, Support at Home, QCSS and NDIS.
- Help clients and carers navigate systems, provide informed consent and share feedback.
- Collaborate with internal/external clinicians for complex needs and referrals.
- Keep accurate records and report outcomes to funding/quality frameworks.
- Build strong, respectful relationships with clients, families and community stakeholders.
- Travel to outreach locations (road and occasional small aircraft) as required.
You’ll Bring
- Experience in case management or coordination in aged care and/or disability services.
- A client-centred, strengths-based approach and cultural humility (especially with Aboriginal and Torres Strait Islander peoples).
- Confidence with mobile tech (smartphone/tablet/laptop) and digital health records; MS Office proficiency.
- Strong communication, documentation and time-management skills.
- Ability to work autonomously and within a multidisciplinary team.
Nice to have (one or more):
- Familiarity with Support at Home, Home Care Packages, CHSP, QCSS, NDIS.
- Clinical background (e.g., RN or Allied Health) for the Clinical Care Partner stream.
Mandatory Requirements
- Current First Aid & CPR.
- National Police Check and NDIS Worker Screening (or ability to obtain).
- Clinical Care Partner only: Current AHPRA registration (discipline-relevant).
