As the Transition Pathways Officer, you will work across a cluster of State High Schools within the Sunshine Coast area to provide a broad range of services aimed at maximising the transition outcomes of students at risk of not successfully completing Year 12.
The Transition Pathways Officer reports to the Manager - Education Justice Initiative within the Student Child Family Connect Team at the Maroochydore Regional Office, as well as to a selection of school Principals.

About the role
As a Transition Pathways Officer, you will:
* Support schools to interpret, develop, refine and enact departmental policies, practices and procedures relating to students exiting school prior to the completion of Year 12 to ensure they remain in an eligible option.
* Provide support to schools that will assist in the development and implementation of successful retention and transition strategies and programs that will improve the transition outcomes for 'at risk students.
* Provide appropriate support through the identification of student needs options and intervention to young people and their families to assist in the transition of young people into a viable education, transition, or employment
* Collate and analyse confidential information, identifying needs and barriers to formulate the basis for the development of a school and post school transition plan for young people (post compulsory participation phase) who leave school before the completion of Year 12 ( transition of students in the compulsory participation phase to be determined by region) and provide appropriate case management, advocacy and support as required.
* Forge links with providers of employment services, community agencies, training organisations and employers to create opportunities for young people to engage in programs that support successful transition from school into employment or training.
* Establish and maintain effective networks with school based support staff across a cluster of schools to secure provision of support and interventions as necessary to create scaffolding that is unique to the demographic and nature of individual case management.
* Utilise formal and informal partnerships with business, industry, education and training providers, parents and other key stakeholders to facilitate casework support information and work readiness decisions for individual student pathways.
* Work directly with schools, government, industry, business and community groups to form productive partnerships and develop networks, operationally both individually and as a member of a work group to facilitate the placement of early school leaders into full or part time employment, training courses or access to employment support services.
* Maintain a reporting framework, and a report on the progress of identifies at risk students to individual school principals and/or their nominee as well as reporting to relevant regional staff.
* Manager partnerships with providers of employment services, transition programs and employers to maximise the employment and training opportunities for secondary students who exit school before the completion of Year 12.
