We are seeking an experienced Youth Mentor to support young people aged 10-25 with NDIS plans. This role involves empowering them to build resilience, develop life skills and reach their full potential.
The ideal candidate will have a genuine passion for helping young people thrive, strong communication and interpersonal skills, reliability, empathy and a collaborative approach.
 * Key Responsibilities:
 o Lead and facilitate group development, coordination and delivery of NDIS-funded group programs.
 o Provide one-on-one mentoring to support young people in achieving personal goals.
 o Evaluate programs and contribute to continuous improvement.
 o Support young people experiencing mental health challenges or disability.
 o Facilitate group activities promoting independence, social inclusion and skill development.
 o Collaborate with colleagues, families and other community services.
 o Maintain accurate case notes and reporting requirements.
