TCP/GEM/Rehab in the Home — locum position in regional Victoria.
Overview
Experienced Social Worker required for a locum based in regional Victoria. Join a team of Allied Health Clinicians, including many of my other candidates who give positive feedback.
Responsibilities
- Caseload includes Transitional Care Program / Rehab in the Home / GEM Program. This is the subacute program that the service discharge patients to for approximately 12 weeks of community follow up by a multidisciplinary team. Social Workers might be working with external providers, the My Aged Care system, liaising with family supports, exploring long term accommodation options, other miscellaneous social issues and general patient support and education
- Knowledge of Victorian and NSW Aged Care system preferred
- Join one other senior Social Worker on the team and an overall allied health team of approximately 10 across these programs
- A driving license is required
- 5 days per week, Monday to Friday
- Free accommodation provided and one way travel covered
Duration & Start
ASAP start for 8 weeks with a possible extension.
