We're looking for an experienced Care Planner / Care Partner who knows the system, understands clients, and can confidently manage care from start to finish.
The Role
You will take ownership of a growing caseload and support clients across Support at Home, ensuring their care is well-planned, well-coordinated, and responsive as needs change.
This is a role for someone who enjoys building relationships, thinking on their feet, and delivering care that actually makes a difference.
In this role, you will:
* Work closely with the leadership team to shape what care support really looks like
* Develop and formalise individualised care plans and service agreements with clients and family where appropriate
* Developing budgets with clients, utilising funding to meet their needs effectively.
* Planning and coordinating the delivery of scheduled services such as personal care, meal preparation, cleaning, and transportation.
* Monitoring the implementation of care plans and adjusting as needed.
* Monitoring clients funding, taking corrective action to ensure clients services are provided within funding and support the management of any unspent funds.
* Conducting regular reviews and reassessments as needed to ensure that clients' needs are being met effectively.
* Providing information and support to clients and their families regarding available services and resources.
* Liaising with healthcare professionals, service providers, and community organisations to ensure coordinated care.
* Maintaining accurate records of Care management services, and documenting client interactions and service provision.
* Maintain clear, compliant documentation
* Adjust care proactively as client needs evolve
About You
You've worked in aged care long enough to understand how it really works — and how to make it work well for clients.
You'll bring:
* Solid experience in Home Care Package, Support at Home, with exposure CHSP being ideal
* Confidence managing your own caseload and making day-to-day decisions
* Strong care planning, coordination, and budgeting skills
* The ability to build trust with clients and families quickly
* A calm, organised approach to managing competing priorities
* A qualification in Aged Care or Nursing - minimum of a Cert III required, Diploma or Nursing qualification preferred\"
