Senior Registered Nurse - Clinical Care Partner
About the Role:
This is a senior nursing position that blends advanced clinical practice with mentorship, advocacy, and strategic care coordination. The successful candidate will be responsible for providing expert clinical guidance and leadership to their team, managing complex clinical cases, and coordinating multidisciplinary care.
Key Responsibilities:
* Providing Expert Clinical Guidance: Develop and implement individualized care plans that address the unique needs of clients with complex health requirements.
* Managing Complex Cases: Assess, plan, and coordinate high-level care services for clients requiring hospitalization or ongoing medical treatment.
* Coordinating Multidisciplinary Care: Work collaboratively with healthcare professionals to ensure comprehensive care delivery, including medication management, wound care, and palliative support.
* Mentoring and Advocacy: Provide guidance and support to colleagues, promoting best practice and evidence-based care standards.
Requirements:
* Registered Nurse Qualifications: Current registration with AHPRA as a Registered Nurse (RN) is essential.
* Leadership and Mentoring Experience: Proven track record of leading teams and mentoring staff in aged care settings is highly desirable.
* Clinical Skills and Knowledge: Possess advanced clinical assessment and case management skills, with experience in palliative, dementia, or end-of-life care.
* Education and Certification: Hold a current First Aid Certificate and possess a strong understanding of Aged Care Quality Standards and clinical governance principles.
