We are seeking a skilled and dedicated Care Manager to join our team. The successful applicant will be responsible for providing high-quality care management and service coordination activities within the Support at Home (SAH) program.
Care Partners work collaboratively with participants to plan, manage, and coordinate services aligned with their aged care assessments and individual goals. They are responsible for delivering person-centred care planning that reflects each participant's unique needs, goals, and preferences.
