Listening and giving people a voice is central to our work. Our support is designed around people's individual needs, choices, and what matters to them. We promote the mentoring value of lived experience through meaningful coproduction and intervention-based support
As a Healthy Together Peer Support Worker you will work in a small team to deliver emotional and practical support through personalised support planning to people living with severe mental Illness (SMI, which includes schizophrenia, psychosis, and bipolar disorder) using your own lived experience to connect with people and to help them achieve their health and wellbeing goals.
You will give people a sense of hope and wellbeing, supporting them to gain a sense of control over their lives, helping them to attend essential healthcare appointments, promoting wellbeing and building connections within their local communities. Providing a trusting, safe, non-judgmental, and respectful relationship to people following their SMI annual health check (AHC) you will:

* Promote equality and diversity, addressing health inequalities to support people to live Equally Well.
