Responsibilities
* Assess patients and deliver clinical treatment and care to patients primarily throughout Wodonga Hospital, the Emergency Department and other wards.
* In partnership with the Medical team, patients, families and carers and relevant agencies develop treatment plans and service delivery frameworks to promote optimum outcomes for consumers.
* In consultation with the patient, family and carers and other care providers, formulate, negotiate, document, monitor and maintain an individual service plan for each patient of the service and complete other documentation, including outcome measures as per service policy.
* Actively communicate and implement the comprehensive treatment plan to the patient and provide education and support to families, carers and clients of the service.
* Identify, engage and include other key community agencies and service providers involved in the provision of rehabilitation, recovery, support and undertake discharge planning consistent with the patient and family/carer needs to minimise the risk of relapse.
* Participate in consultation, awareness and education activities under the supervision of the AOD NP, develop resources to increase the understanding of substance use disorders and service delivery models for other clinical teams within the health service.
* Support the AOD nurse responsible for the Wodonga Hospital withdrawal bed.
