Central Coast Local Health District is looking for caring and compassionate Health Clinician Level 3 for Older Peoples Care Coordinator roles at Gosford. The successful candidate will possess specialist knowledge regarding mental health specific assessments, clinical treatments, and be able to co-ordinate care for individuals with complex needs.

About You:
* You have a minimum Tertiary Bachelor Degree in Psychology, Occupational Therapy with AHPRA registration or Social Work with AASW membership or Counselling or Community Welfare, with three years post-graduate experience in mental health; OR Master's Degree Clinical Psychology with endorsement to practice as a Clinical Psychologist with AHPRA OR Tertiary Bachelor's Degree in Nursing (Inc. Div. 1 AHPRA registration) with relevant post-grad qualifications and at least 3 years' experience working in the clinical area of their post-grad study.
* You possess demonstrated knowledge of working within a care co-ordination model for Older Persons with complex needs and sensitivity to the mental health needs of older persons and their families.
* You have demonstrated ability to work independently and within a multidisciplinary team and experience working with a range of community partners including General Practitioners.
Where you'll be working:
The position sits within the Older Person’s Mental Health Team which follows a recovery model of care as per The National Framework for Recovery for Mental Health Services. The OPMH team provides care co-ordination services to persons over the age of 65 years who have a moderate to severe mental illness, living within the community setting including residential aged care facilities.
The primary role of the OPMH Care Co-ordinator (Level 3) is to work as part of a team that provides care co-ordination, case management, family/carer interventions, recovery focused interventions. This position requires the position holder to utilise advanced reasoning and clinical skills to support patients with complex needs. The position provides specific mental health assessments, clinical treatments, and referral in accordance with relevant mental health guidelines and best practice standards.
