We are seeking a dedicated Care Coordinator to join our team, providing quality in-home care and assistance to our clients. The successful applicant will work closely with our care team to ensure services are delivered in accordance with client needs, goals, and preferences.
Key Responsibilities
* Building strong relationships with clients and their families to deliver personalized support
* Assigning staff to clients based on qualifications, availability, and client preferences for effective service delivery
* Maintaining accurate records of client progress and outcomes, including any incidents or feedback received
* Providing referrals to external agencies that can offer additional support to meet client needs
* Assessing client requirements and resources to identify gaps in health, welfare, and other essential services
* Responding promptly to client inquiries, requests, and emails to ensure excellent communication
* Maintaining up-to-date knowledge of relevant policies, procedures, and best practices in the field of care coordination
The ideal candidate will possess experience in assessing and planning care needs, a thorough understanding of the Aged Care and Disability systems, and excellent organizational, administrative, and time management skills.
