ACM Healthcare is working on behalf of a Federal Government Department to source an experienced Mental Health Skilled for a Clinical Care Coordinator role, providing Case Management services to Veterans and their Families. This is a two-year contract position.
$67.24 - $76.36 an hour + Super

Open to Psychologists, Social Workers, Mental Health Nurses, Occupational Therapists and Counselors
Position available in Hobart, Launceston, Devonport, or Burnie office
Duties
* Deliver high-quality, evidence-based clinical services, including risk assessment, brief interventions, care coordination, and referrals for clients with complex/multiple needs.
* Provide short‐term, goal‐oriented, clinical case management intervention to clients who present with unmet health and psychosocial needs that impact on their ability to access and/or meaningfully engage in mental health care.
* Maintain accurate and confidential client records, including progress notes, care plans, and risk assessments.
* Collaborate with internal teams and external organisations to ensure coordinated, client‐centred support and effective referral pathways.
* Work within a stepped‐care model, supporting vulnerable or at‐risk communities and families.
* Participate in skilled development and supervision to maintain registration and uphold best‐practice standards.
* Contribute to service improvement initiatives, program delivery, and community engagement activities as required.
Mandatory Requirements
* Qualification as a registered Psychologist / Mental Health Occupational Therapist / Mental Health Registered Nurse with full AHPRA registration, OR
* Qualification as a Social Worker with membership of the Australian Association Social Workers (AASW) on commencement OR
* A Masters‐level counselling qualification and registration with PACFA as a Registered Clinical Counsellor or with ACA at Level 3 or Level 4.
* Working with Children (WWC) and/or Vulnerable People Checks/Clearance must be obtained prior to commencement.
* As this role is within a Federal Government department it is required to be an Australian Citizen at time of commencement.
