The Role
As a Support Coordinator based in the Goulburn Valley (Shepparton, Euroa, Cobram), you will:
* Provide high-quality, person-centred Support Coordination to NDIS participants.
* Help participants understand and implement their NDIS plans.
* Strengthen participants' capacity to navigate health, mental health, housing, justice, and social services.
* Conduct face-to-face meetings, case conferences, and oversee NDIS budget management.
This role reports directly to our Shepparton Business Unit Manager. Flexibility and local connection are key—you must live in the region to be considered.

About You
We're looking for a confident, community-minded professional who brings:
* A Bachelor qualification in Social Work, Community Services, Health Science, Psychology, Case Management, or related field (Diploma considered).
* Proven experience working with people with disability, mental health, or psychosocial needs in case management or a similar role.
* Strong knowledge of disability, mainstream and community services.
* Excellent communication and report-writing skills.
* A valid Driver's Licence, reliable vehicle, and current probity checks (WWCC, NDIS Worker Screening, Police Check).
