Journey Cordinator - MyndKind

* Location: Mackay, Whitsunday, Issacs Regions
* SCHADS Award-Level 4 ($88,090.08 - $94,788.72)
* Maximum Term Full Time contract until 30/06/2026

POSITION DETAILS

Journey Coordinators work within the MyndKind service system, providing independent support to guide individuals through their care journey. Working as a standalone service or alongside Front Door and other MyndKind programs, the role delivers person centred, collaborative care coordination to improve access, service integration and mental health outcomes.

KEY RESPONSIBILITIES

Participant Support

* Deliver person led, strengths based psychosocial support.
* Use the IAR DST to assess eligibility and make appropriate referrals.
* Codesign participant goals to build capacity and support recovery.
* Conduct risk assessments and develop safety plans as needed.
* Respond safely and professionally to suicide risk, distress and challenging behaviours.

KEY SELECTION CRITERIA

* Ability to build genuine, respectful and trusting relationships.
* Strong rapport building skills using recovery focused, person centred and trauma informed practice.
* Solid understanding of mental health challenges and support needs.
* Knowledge of local services, or the ability to quickly learn regional referral pathways.
* Ability to meet deadlines, manage workload and exercise sound judgement.
* Current QLD C Class Drivers Licence.
* Federal Police Check, Blue Card and NDIS Worker Screening (Yellow Card) required.

DESIRABLE

* Current First Aid and CPR certificates.
* Certificate IV or Diploma in Mental Health.
* Relevant qualifications or understanding of mental health theories/frameworks.
* Skills in counselling and motivational interviewing.
* Lived experience of holistic recovery.
