Job Opportunity

We are seeking a dedicated professional to fill the role of Youth Health ED Care Navigator. This position plays a vital part in enhancing access to healthcare for marginalized young people, focusing on those who frequently present to Emergency Departments.

* This role involves supporting marginalized young people to better navigate the health care system.
* The ideal candidate will focus on enhancing access to primary and community health services.
* Key responsibilities include reducing reliance on Emergency Departments by improving care pathways.
* The Youth Health ED Care Navigator will work collaboratively with other service providers to empower young people through improved health access.


Responsibilities

1. Navigating Health Systems: Support marginalized young people to better understand and utilize health services.
2. Enhancing Access: Focus on improving access to primary and community health services for this vulnerable cohort.
3. Reducing Reliance: Collaborate with stakeholders to develop care pathways that reduce reliance on Emergency Departments.
4. Building Relationships: Foster strong, trust-based relationships with young people facing barriers to care.
5. Addressing Gaps: Identify and address systemic gaps in access to ongoing, coordinated health support.
6. Collaboration: Work with stakeholders to develop a framework that improves access for this cohort.
7. Strategic Planning: Contribute to long-term strategies that improve equity and health outcomes for vulnerable youth.


Requirements

* Demonstrated understanding of and experience working with young people from marginalized and at-risk groups.
* Collaborative experience working with General Practitioners, Community Services, Non-Government Organisations, and multidisciplinary teams.
* Knowledge of NSW Health initiatives for Youth Health, NSW Government Youth Initiatives, and Primary Health Care principles.
* High-level communication, liaison, negotiation, time management, computer, and presentation skills.
* Knowledge of Acute service settings or research & evaluation experience, including data collection and data entry.
