Job Title: Family Support Worker
Family Support Workers are pivotal professionals who provide non-clinical therapeutic support to individuals, families and groups facing social, emotional and/or financial challenges.
This permanent Full Time role is based in Caboolture.
Key Responsibilities:
* The position involves conducting intake assessments including risk and safety planning for effective service delivery.
* Developing and delivering personalized family support plans with suitable referrals to meet unique client needs.
* The role requires providing outreach services such as school or home visits to cater to diverse client requirements.
* The Family Support Worker will be expected to facilitate skill development among clients and strengthen their relationships through active engagement.
* Servicing this position also entails liaising closely with key community agencies, such as child protection, youth justice bodies, etc., for better coordination of care delivery.
