- About the role
We are seeking a dedicated Case Manager for our Homelessness Stream! You will provide case management outreach and support to people who are homeless or at risk of homelessness to access safe, affordable accommodation and prevent their return to the homelessness service system.
Responsibilities include
- Case management support is provided to clients in accordance with TSA case management models and standards of practice
- Individual case, safety and or private rental action plans are developed in collaboration with clients and include a range of interventions that address the needs, risks, and goals of clients
- Risks to clients, staff and TSA are identified and bought to the attention of Management as soon as possible. Relevant documentation is completed in accordance with WHS policy.
This is a permanent full-time position based in Leongatha, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.
- Relevant tertiary qualification in social work, community services, youth work, psychology or community development - diploma level (minimum)
- Knowledge of theories of human development and how they impact on current day approaches and interventions in the social services sector
- Demonstrated knowledge of theoretical frameworks that underpin attachment theory and trauma informed practice
- Expertise in working with clients with multiple vulnerabilities, histories of trauma, and complex needs
- Current and valid Driver’s Licence is required
