Purpose of the Position
The Caseworker's primary role is to support the Team Leader to achieve client engagement and client outcomes.
Responsibilities and Duties
* To provide front line case management to achieve better outcomes in accordance with the goals of Aboriginal and Torres Strait Islander clients
* Build respectful and professional relationships with service users, agencies, partners, community, colleagues and clients
* Participate in assessment, case planning, organisation and delivery of service provision and interventions, reporting and engagement for clients
* Follow Assessment, Case Management and Reporting procedures and standards and contribute to continual improvement of services
* Care coordination - Implement Case Plans, including advocating for, supporting and journeying with the client to achieve their desired outcomes
* Participate and contribute to the organisation's compliance obligations under the ISO 9001:2015 Quality Management System and Human Services Quality Standards (HSQS)
* At all times work within CQID's Workplace Health & Safety Policies and Procedures
* Other responsibilities as directed by the Team Leader
Qualifications and Experience
* Holds a current C Class Queensland Driver's Licence
* Holds or is willing to obtain a First Aid and CPR Certificate
* Knowledge of the Community Services Act 2017 and Commonwealth Supported Accommodation Assistance Act 1987
* Experience at successful service delivery to Aboriginal and Torres Strait Islander people
* Experienced in client engagement and case collaboration
* Experience in working with individuals who are experiencing drug and alcohol issues
* Current Working with Children Check, "Blue Card"
Skills and Attributes
* Aboriginal and/or Torres Strait Islander Heritage-–[Note: Non-Indigenous applicants will only be considered if no suitable Indigenous candidates are attracted in the first round of applicants]
* Competent in use of Microsoft Office software
* Physical ability to undertake job responsibilities
* Ability to communicate effectively; spoken and written
* Self-starter, Motivated and able to self-manage work tasks
* Strong connection to and knowledge of local Indigenous Community
