Overview
Redeemed care is seeking to employ 2 Community Worker who will assess client needs, develop support programs, coordinate services and advocate on behalf of individuals and families experiencing social disadvantage, disability or vulnerability. The role requires professional case management, referral coordination and compliance reporting in accordance with NDIS and community services legislation.
Responsibilities
* Interviewing clients to assess social, emotional and practical support needs
* Conducting assessments and developing individualised support plans
* Providing case management and ongoing client monitoring
* Coordinating referrals to health, housing, education and welfare services
* Advocating for clients' rights and access to services
* Facilitating community participation and social inclusion programs
* Preparing case notes, progress reports and funding documentation
* Liaising with government agencies and community organisations
* Supporting crisis intervention and risk management processes
* Participating in case reviews and multidisciplinary meetings
* Ensuring compliance with organisational policies and safeguarding frameworks
Essential Requirements
* AQF Diploma or higher qualification in Community Services, Social Work or related field
* Minimum 1 year relevant work experience in community services or related
* Demonstrated experience in case management and client assessment
* Knowledge of NDIS framework and community services legislation
* Strong written documentation and reporting skills
* Current National Police Clearance
* NDIS Worker Screening Check
* Current First Aid & CPR
* Valid WA Driver's Licence
Employment Conditions
Full-time position. Some travel and flexible hours may be required.
