We are looking for dedicated Home Community Support Workers to provide high-quality, person-centered support to NDIS Participants. Your role will be crucial in helping them achieve their goals, enhance their quality of life, and improve their physical and mental health.Key ResponsibilitiesAs aHome Community Support Worker your responsibilities will include:Individualised Support: Providing a range of services tailored to meet the unique needs of NDIS Participants, such as housekeeping, grooming, toileting, meal preparation, medication management, and more.Knowledge and Care: Demonstrating an understanding of health and well-being, cultural plans, transition plans, and behavior support plans. You will also be knowledgeable about protocols regarding restrictive practices and medication requirements.Community Inclusion: Assisting Participants in participating in community activities and services based on their preferences and abilities. You will help them establish and maintain relationships and interests.Communication: Providing regular feedback to the Home and Community Support Manager regarding issues that require follow-up.
