Targeted Care Package Case Manager

We're seeking an experienced and compassionate Case Manager to join our team and deliver Targeted Care
Packages (TCP) for young people at risk of entering residential care and/or transitioning from residential care.

The Opportunity:
We're seeking a compassionate and skilled Targeted Care Package Case Manager to join our dedicated
team. This role plays a vital part in supporting young people by providing creative, specialised placement
and support options for young people with complex needs and behaviours. You'll work collaboratively with
clients, carers, families, community supports, and key stakeholders to ensure that each young person
receives tailored, holistic, and trauma-informed support that promotes independence, wellbeing, and
positive long-term outcomes.
About You:
You're an empathetic and motivated professional with strong case management experience in child, youth,
or family services. You thrive in a dynamic environment and are confident managing multiple priorities while
maintaining a client-centred focus.
You will bring:
• Tertiary qualification in Social Work, Psychology, Youth Work, or a related discipline.
• Experience delivering case management or care coordination within the child, youth, or family
sector.
• Knowledge of trauma-informed practice and an understanding of the needs of children in out-ofhome care.
• Strong interpersonal, advocacy, and report writing skills.
• Understanding of DFFH frameworks, Targeted Care Packages, and child protection systems
(desirable).
• A current driver's licence, Working with Children Check, NDIS Worker Screener Check and National
Police Check.
