Case Manager Job Description
This role involves providing culturally secure case management for clients and their family members through advocacy, follow-up, referral to appropriate services, and collaboration with other support agencies.
The primary responsibility of the Case Manager is to facilitate a smooth hospital discharge for our clients by completing necessary documentation and assessing for any ongoing needs and goals.
* Experience in providing culturally appropriate support services, including case management, counseling, advocacy, and supports to Aboriginal people is essential.
* Understanding of privacy and confidentiality issues is a must.
