NDIS Support Coordinator/ Recovery Coach

At KonnectUs, our highly experienced team of Support Coordinators works collaboratively with NDIS participants to navigate the NDIS system, connect with appropriate service providers, and achieve meaningful goals. We coordinate a wide range of services—including allied health, personal care, employment support, housing, and community access—by engaging with NDIS providers, mainstream services, government departments, and private health professionals.
We have developed a particular strength in working with participants who experience psychosocial disabilities, supporting them with compassion, consistency, and trauma‑informed approaches. Our team has the knowledge and experience to assist individuals with mental health challenges to build capacity, increase independence, and improve their overall well‑being. We are skilled in managing complex situations, including crisis planning, hospital discharges, homelessness, and recovery‑oriented practice.
Key Responsibilities
* Support implementation of the participants' NDIS plan
* Ensure mainstream services meet their obligations (i.e. Therapists, housing, education, justice, health)
* Build the capacity of the participant to achieve greater independence to self‑direct services and supports in the longer term
* Provide the NDIA with reports on outcomes and success indicators within the agreed reporting frequency
* Understand the Plan and Design Support Approaches
* Connect with Supports and Services
* Coach, Refine, Reflect and provide feedback
* Crisis point: Planning, Prevention, Mitigation and Action
Required Knowledge, Skills & Experience
* Tertiary qualification in relevant field (e.g., Disability, Social Work, Psychology)
* Strong written and computer skills for the creation of plans, programs, reports, correspondence and other documents
* Experience working in disability, mental health, community services, or case management (NDIS experience highly regarded)
* Strong understanding of the NDIS, psychosocial disabilities, and trauma‑informed care (desirable)
* Excellent communication, organisational, and interpersonal skills
* Cultural competence and a commitment to inclusive practice
* Ability to work independently and manage a diverse caseload
* Valid driver's license and access to a reliable vehicle
* Current Working with Children Check, NDIS Worker Screening Check, and National Police Clearance (or willingness to obtain)
* Strong coordination, leadership and peer support skills
* Ability to communicate in more than one language
* Knowledge of MYP Software
* Relevant Lived Experience strongly considered
* Internal and External Clinical Supervision provided
