Job Description
Position Summary:
* This position is responsible for delivering patient-centered care within the WA Home Therapies setting in accordance with organisational core values, standards of practice, and guidelines set out by APHRA.
* Provides support to the management team by participating in clinical management, education, and quality activities for staff and patients within the training units.
* Works autonomously within the centre, following accepted practice standards, with guidance and support from level 2 CNs and the WA Home Therapies Manager.
* Participates in the design, planning, delivery, and evaluation of patient care within the home dialysis therapies clinical setting, including community visits across WA. This includes training patients and carers to manage peritoneal dialysis or haemodialysis.
* Protects patient rights and maintains confidentiality in accordance with Privacy Principles.
* Acts as the primary coordinator of care for a group of dialysis patients, collaborating with other healthcare team members in tertiary and community health settings.
The role also involves providing education and support to clients to facilitate the maintenance and restoration of their health and well-being.
* Provides education, support, and guidance to junior staff, including enrolled nurses, and works as part of a team to deliver high-quality care.
* Participates in staff education, serving as a preceptor and role model as required.
* Acts as a positive team member, promoting the centre's values, mission, and goals, working towards their achievement.
* Ensures a safe, secure, and healthy working environment as outlined in the Occupational Health and Safety Site Safety Plan.
* Engages in quality improvement by reporting incidents and hazards and assisting in identifying opportunities for improvement, such as audits.
* Participates in professional development activities through internal and external education programs to maintain and update clinical knowledge and skills.
* Completes annual clinical and mandatory competencies.
* Performs management functions when requested by the WA Home Therapies Manager or Clinical Coordinator.
* Maintains accurate patient records, including electronic records.
