About the Role
You will work in a dynamic and supportive environment, developing individualised plans for customers with disabilities or older Australians to live and recover within their community.
* Develop person-centred support plans in collaboration with customers.
* Provide transportation, social activities, financial assistance, and personal care to customers.
* Encourage customers to participate in activities of their choice, promoting independence and well-being.
The ideal candidate will have a Cert III in Disability, Mental Health, Peer Work, Individual Support, or equivalent, and hold a valid NSW Drivers License. You must also have an Australian Police Check, NSW WWCC, and NDISWC or willingness to obtain before commencing the role.
A strong set of values guiding your work, incorporating our organisational values of Passion, Integrity, and Respect, is essential. Great computer skills, particularly with Microsoft Office and customer relationship management systems, are also required.
