The Clinical Nurse Consultant - Palliative Care provides clinical leadership and direction in planning, implementing and evaluating the provision of palliative care to effectively support the delivery of a community palliative care service provided by the Primary Health North West Area Services.
Team and context
The Specialist Palliative Care Service provides consultative, multidisciplinary support to patients, carers, and clinicians across both community and inpatient settings in North West Tasmania. As part of a small, dedicated team, you'll contribute to delivering best-practice care that prioritises dignity, comfort, and individual choice.
The Role
* Provide leadership within the team in supporting palliative clients in a variety of locations, including home visits to patients and families; assess conditions and needs for supports and report back to the specialist team.
* Collaborate with the wider team to ensure timely response to requests for support and assess response to interventions.
* Demonstrate excellent communication and documentation skills.
Qualifications and requirements
* Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse (essential)
* Current Driver's Licence (essential)
* Excellence in communication across professional and client settings (essential)
* Experience and understanding of the palliative approach to care (essential)
* Demonstrated ability to work collaboratively across acute and primary health settings with THS and NGOs to ensure comprehensive palliative care (essential)
* Desirable: holds or is working towards relevant postgraduate tertiary qualifications
Details of appointment
* Permanent, full-time, day worker position, up to 76 hours per fortnight, commencing as soon as you can join us. Hours to be negotiated with the successful applicant.
* Applicants should note that for a period of twelve months from the publication date, the selection process may be used to fill subsequent or similar vacancies.
Salary and benefits
* Salary: $118,297 to $125,543 per annum. Employer 12% superannuation on top of this amount.
