As a contractor with CoAbility, you'll work within our guidelines to ensure we stay compliant with our NDIS registration. Your responsibilities will include: Caseload Management Prioritise and manage your caseload efficiently to ensure timely, participant-focused service delivery. Participant Empowerment Support participants to build capacity, develop self-advocacy skills, and increase their ability to manage their supports independently. NDIS Plan Implementation Assist participants in understanding their plan, connecting with providers, setting up service agreements, and managing budget use. Outcome Tracking & Review Preparation Support participants to reflect on current supports and prepare for plan reviews by tracking outcomes and identifying new goals. Consistent Participant Engagement Maintain regular, meaningful contact with each participant on your caseload — at minimum, every two weeks — or more frequently for higher-risk participants. Collaborative Support Planning Work with participants, families, and providers to co-design support plans that reflect NDIS goals and individual strengths. Timely Record Keeping Accurately document all billable and non-billable activity in our CRM by the close of business on the day of service. Referral and Resource Navigation Identify and connect participants with appropriate supports across funded, mainstream, and informal networks. Crisis Management Respond promptly and professionally to participant crisis situations, including complaints and incident reporting. Quality and Compliance Follow CoAbility's policies and procedures, the NDIS Code of Conduct, and all relevant legislation and practice standards. Administrative Accuracy Ensure all data, documentation, and billing are accurate, complete, and submitted on time to enable smooth service delivery. Team Participation Be an active part of the team: attend supervision, participate in team meetings, and contribute to a positive, inclusive culture. Professional Boundaries & Ethics Maintain professional boundaries and act with integrity and respect in all CoAbility-related interactions, including with participants, colleagues, and stakeholders. You'll need to meet a set of baseline requirements. These are in place to protect everyone—ensuring you're covered, we remain compliant, and participants receive safe, high-quality support under the NDIS. Let's Get Started Qualifications & Experience
* Relevant tertiary qualification (e.g., Social Work, Occupational Therapy, Community Services)
* Minimum two years of NDIS Support Coordination, case management, or transferable experience
