Clinical Care Coordinator – Bishop Tyrell Place, Cundletown

Working alongside the Residential Care Manager, you'll help lead the clinical operations of the home, ensuring our residents receive exceptional care in line with the Aged Care Quality Standards. This is an ideal role for someone with strong change management skills and a genuine passion for uplifting people, culture and care outcomes. The position is full‐time, permanent and may include occasional weekend work. It is based within our Residential Aged Care Home and requires current AHPRA registration and proven clinical experience within the aged care sector.

Role & Responsibilities
* Organise and facilitate case conferencing to partner with consumers and their representatives in the development of care provisions and plans.
* Act as a clinical care leader and go‐to contact within the home.
* Provide expert case management alongside the Residential Care Manager.
* Monitor and improve clinical outcomes through audits and quality initiatives.
* Support policy development and implementation for resident care.
* Lead, coach and inspire staff to deliver outstanding care.
Qualifications & Experience
* Registered Nurse with current AHPRA registration.
* Proven clinical and leadership experience in residential aged care.
* Strong skills in assessments, care planning and change management.
* Sound understanding of aged care funding models.
* Commitment to best practice and continuous improvement.
