This is a non-clinical role focused on care coordination, participant engagement, and service excellence within Commonwealth Home Support Services (CHSP), Support at Home (SaH), or Home Care Packages (HCP). The role supports older people to live independently and with dignity in their own homes across Newcastle and Hunter regions.
About The Role
As a Care Partner, you will build trusted relationships with participants, assess and coordinate support services, and develop holistic care plans in collaboration with participants, their families and carers, and allied health/clinical teams. Your plans will reflect individual goals, strengths and preferences to help participants achieve their goals at home.
This is a non-clinical role focused on care coordination, participant engagement, and service excellence.
Key Responsibilities
- Coordinate care and support services in line with Aged Care Quality Standards.
- Develop goal-oriented care plans and budgets in consultation with participants and stakeholders.
- Provide education and resources to empower participant choice and control.
- Maintain accurate records in My Aged Care and other systems.
- Participate in service development initiatives to enhance wellbeing and quality of life.
- Collaborate with internal and external stakeholders to ensure integrated service delivery.
- Contribute to service excellence, continuous improvement and respond to aged care reforms.
Essential Requirements
- Experience working in aged care across community or residential settings, including participants with dementia.
- A solid understanding of CHSP, SaH or HCP funding.
- A strong collaborator with excellent communication and interpersonal skills.
- Ability to work collaboratively within multidisciplinary teams.
- Excellent planning, time management, and organisational skills.
- Strong decision-making and problem-solving and adaptability.
- Confidence using technology and contemporary systems.
- A commitment to wellness, safety, and continuous improvement.
- Certificate IV or Diploma in Community or Aged Care is desirable.
- Current unrestricted driver’s licence and access to a registered, insured vehicle.
- Current NDIS Worker Screening Check.
- National Police Check.
- Australian work rights.
