Restorative Care Therapist - Community Services
Support older Australians with evidence-based in-home occupational therapy services, focusing on home safety assessments, equipment prescription, and empowering clients to live independently.
* Deliver 1:1 in-home occupational therapy services.
* Focus on Home Care Package and Short-Term Restorative Care clients.
* Engage in mobile assessment, case management, and care coordination.
* Facilitate client connections to appropriate service providers and supports.
Key Skills:
* Passion for working with older adults.
* Excellent organizational and communication skills.
* Proficient in MS Office and client management software.
* Current unrestricted driver's license.
