Community Component: Conducting thorough assessments and reporting on the current needs of community clients upon referral. Devising tailored management plans to address assessed needs and support clients in achieving their goals. Providing expert advice on the optimal frequency of visits and efficiently scheduling consultations with multiple clients. Highly regarded candidates will have experience in community packaged care, bringing valuable insights to our team. Residential Aged Care Facility Component: Collaborating with healthcare staff at aged care facilities to assess and treat new residents upon their arrival. Offering guidance on the frequency of treatments and collaborating with onsite staff to establish streamlined treatment systems. Completing comprehensive clinical notes and assessments at the facility to facilitate the processing of clients' needs.
