The role of a Home Care Support Coordinator is pivotal in delivering high-quality aged care services. In this position, you will be based in Darwin and play a key part in managing the delivery of in-home aged care services to clients.
Key Responsibilities
* Coordinate and manage the delivery of in-home aged care services
* Ensure compliance with relevant legislation, quality frameworks, and funding guidelines
* Work independently with minimal supervision
* Have experience in a Support or Personal Care worker, preferably within the Aged Care industry

Requirements for the role include:
* Certificate IV (or higher) in Aged Care / Individual Care or Disability Care
* A valid C Class Driver's Licence and access to own vehicle with full comprehensive insurance
* Ability to travel to clients' homes when required
* Prior experience in a Support or Personal Care worker, preferably within the Aged Care industry
