Key Responsibilities
The Clinical Community Health Worker is a vital member of the interdisciplinary health team. They are responsible for reaching out to vulnerable populations in the community, conducting basic health screenings and point of care testing, referring patients to healthcare providers and community resources as needed, and providing case management services.
* In collaboration with a peer support specialist, engages with individuals within the substance use community to provide health information and services.
* Provides basic health screenings and point of care testing out in the field, referring individuals to healthcare providers and community resources when necessary.
* Prioritizes connecting with individuals experiencing homelessness and/or substance use, distributing basic supplies, referring them to community resources, providing transportation if needed, and offering case management services.
* BUILDs rapport with community members and identifies opportunities to interact with them to meet their current needs.
* Contributes to planning, organizing, and staffing community health education and screening events.
* May be required to work occasional evenings and weekends.
* Takes basic vital signs and becomes proficient in conducting basic health screenings.
* Serves as an ambassador for Blue Ridge Health, promoting our services to other agencies, employers, community members, and potential clients.
* Collaborates with clinical staff as directed, functioning as a member of the clinical team.
Requirements
* Active licensure as a Medical Assistant, Licensed Practical Nurse or Registered Nurse is required.
* Lived experience in homelessness and/or migrant agriculture work is highly preferred.
* A high school diploma is required.
* A valid North Carolina driver's license and eligibility for coverage under Blue Ridge Health’s automobile insurance policy are necessary.
