Social Worker – Aged Care

About the Role
As a Social Worker at AvantiCare, you will deliver evidence-based psychosocial support to clients and families, supporting emotional, practical, and social aspects of health and wellbeing.
You will work as part of an interdisciplinary allied health team, supporting holistic outcomes through ethical, compassionate and client-centred practice.
Key Responsibilities
* Conduct comprehensive psychosocial assessments
* Provide counselling, crisis support and advocacy
* Develop and implement client-centred support, rehabilitation and discharge plans
* Support clients to access appropriate community and external services
* Promote client dignity, autonomy and informed decision-making
* Collaborate with allied health professionals, families, carers and external stakeholders
* Maintain timely and accurate clinical documentation
* Contribute to quality improvement and service development initiatives
About You
Essential requirements:
* Bachelor of Social Work (or equivalent)
* Current AASW membership
* Experience in health, aged care, disability or community settings
* Strong communication, organisational and time-management skills
* Ability to work independently and as part of a team
* Valid Australian driver's licence and reliable vehicle
* Current National Police Clearance and/or NDIS Worker Screening
* CPR/First Aid certification
Desirable:
* Experience supporting clients with chronic or complex health conditions
* Understanding of aged care funding (HCP, CHSP)
* Familiarity with electronic health record systems
