The opportunity exists for a nurse with both District Nursing and Palliative Care experience to join the community services team for a fixed term of 4 weeks to cover leave.
This role of the Community Services associate manager is to support the Community Services Manager with the day to day running of both District Nursing and Palliative Care departments.
The fixed term contract is from Monday 13th April until Sunday 10th May 2026.

About the position
We are seeking a Community Services Associate Manager join us on a Full-time fixed Term basis.

What you'll do
Assess and triage referrals to the service
* Telephone liaison for both new and existing Community Services clients
* Act as a resource and role model to other staff
* Rostering and daily staff management
* Review and update relevant policies as requested
* Conduct Quality Activity Audits and completion of quality data reporting as requested
* Undertake urgent home visits as needed
* Communicate and collaborate with other stakeholders
* Demonstrate advanced clinical knowledge and skills
* Demonstrate knowledge and promote the use of advanced care planning for
residents/clients/patients
* Contribute to the strategic direction of Community Services Programs and Seymour Health
What you need
* Understanding of the Aged Care Act
* Understanding of the Palliative care, NSQHS, NDIS and Strengthened Aged Care standards
