Mounts Medical Centre is seeking a motivated andexperienced Advanced Nurse Practitioner ANP to join our excellent andsupportive clinical team on a full-time basis.
You will be working within a highly collaborativeenvironment alongside two other ANPs with full clinical support from anon-call GP, five GP partners, one salaried GP, and two experienced practicenurses. Our multidisciplinary team is committed to delivering high-quality,patient-centred care in a friendly and professional setting.
Main duties of the job
Assess, diagnose and manage a wide range of acute andchronic conditions
Provide evidence-based clinical care within your scope ofpractice
Prescribe independently where qualified
Work collaborativelywith GPs, nurses and the wider practice team
Conduct home visits whenrequired, with full clinical support from the on-call GP
Contribute toservice development and continuous quality improvement
Maintain accurate andcontemporaneous clinical records
