We are seeking a skilled and compassionate Client Coordinator to join our team. As a Client Coordinator, you will be responsible for meeting with clients and their families to assess their needs and build strong connections. You will conduct client care assessments and prepare care plans to best meet client requirements. Additionally, you will provide some nursing care and in-home care services as needed.

Required Skills and Qualifications:
">
* A minimum of 1-2 years of experience in community nursing with current AHPRA registration is preferred
* Cert III/Cert IV in Aged/Disability Care with a minimum of 4 years of experience is also acceptable
* Experience in the aged care/disability/mental health sector in a community setting is essential
* Understanding of Aged care & Disability standards and consumer directed care is required
* First Aid, current CPR, and National police check are necessary qualifications
* WWC (or willingness to obtain) and a driver's licence with reliable car and comprehensive insurance are also required
* High level of computer literacy and understanding of clinical governance and continuous improvement are desirable skills
