Job Description
Job Summary
Wise Healthcare is seeking compassionate and reliable Disability Support Workers to provide high-quality, person-centred care to individuals living with disabilities across various suburbs in the Melbourne region. This role is crucial in assisting NDIS participants with daily living activities, promoting independence, and facilitating meaningful community engagement. As a Disability Support Worker, you will be instrumental in helping participants achieve their NDIS goals, enhance their quality of life, and build confidence in their everyday activities.
About the Role
We are looking for motivated Disability Support Workers to join our Disability Services team and provide direct support to NDIS participants in their homes and within the community.
The role involves delivering person-centred support tailored to each participant's individual needs, helping them develop independence, build life skills, and participate actively in community life.
Support Workers will work closely with participants, families, support coordinators, and allied health professionals to ensure consistent and high-quality care aligned with individuals' support plans.
Key Responsibilities
Participant Support
* Provide person-centred support tailored to participant needs and goals
* Assist participants with daily living activities including personal care and hygiene
* Support participants in maintaining independence and dignity
Daily Living Assistance
* Assist with meal preparation, household tasks, and daily routines
* Provide mobility assistance where required
* Support medication reminders where appropriate
Community Participation
* Support participants to engage in community, recreational, and social activities
* Assist participants in attending appointments and community events
* Encourage social inclusion and confidence building
Life Skills Development
* Support participants in developing life skills and independence
* Encourage participation in education, employment, or community activities
* Promote choice and control in decision-making
Documentation and Reporting
* Maintain accurate progress notes and support records
* Report changes in participant behaviour, wellbeing, or needs
* Complete incident reports and documentation where required
* Work collaboratively with families, carers, and healthcare professionals
* Follow individual support plans and behaviour support strategies
* Contribute to a supportive and professional team environment
Requirements
* NDIS Worker Screening Check
* National Police Check
* Working With Children Check (Employee)
* First Aid and CPR Certification
* Completion of NDIS Worker Orientation Module
* Strong communication and interpersonal skills
* Ability to work independently and within a team
* Commitment to person-centred care and participant wellbeing
* Previous experience in disability support or community care
* Certificate III or IV in Disability, Individual Support, Aged Care, or related qualification
* Experience supporting participants with complex support needs
* Training in manual handling or medication administration
* Experience working within the NDIS sector
Compliance Requirements

* NDIS Worker Screening Check
Work Entitlements
* Candidates must have appropriate work entitlements to work in Australia
