About us:

CCA NSW is a busy, non-profit community organization, providing services to frail aged/ disability clients for over 60 years. We are seeking a vibrant, collaborative, detail-oriented worker to support elderly people and people with disability to continue living in their own homes while maintaining their independence and connection to the community. This is an exciting opportunity for someone looking to learn and grow in both the NDIS and Aged Care Sectors.

About the role:

This is a Full-Time position (Part Time negotiable) based in our Eastwood office. Your new role, as an Aged and Disability Services (ADS) – Client Care & Compliance Coordinator, entails supporting both NDIS and aged care clients to access and engage services that build their capacity and maintain independence and individuality, Coordinating support workers, Ensuring compliance to government regulation and Promoting your services.

Qualifications & Experience

A Bachelor degree in relevant field, Diploma on Community Services, Individual Social Support Certificate IV or Nursing, or extensive, relevant experience
Demonstrable ability to work in a team and independently
Extensive knowledge of the community sector, predominantly in NDIS and Support at Home
Experience in government funding/reporting and compliance requirements (CHSP, Support at Home, NDIS)
Experience with Rostering (Employment Hero)
Networking skills
Knowledge of evidenced-based and outcome driven programs
Excellent written and oral communication skills.
Ability to effectively interact with people at all levels and from diverse backgrounds.
Drivers license and access to a car.
Strong time management skills, commitment to meeting deadlines and the ability to cope effectively with a sometimes pressured working environment
Intermediate computer skills – Microsoft Office (Word, PowerPoint, Excel).
Demonstratable problem and data analysis skills
Bilingual skills will be a benefit

Tasks & Responsibilities

To ensure the effective running of ADS services providing background support including ensuring compliance.
Assist Manager of ADS with administration compliance and future planning.
Coordinate CCA's Individual Social Support and Medical Transport programs, including scheduling support workers.
Provide on-call support when required, e.g. overnight and/or weekend.
Drive client acquisition campaigns.
Intake, on board and assess clients.
To collect data and maintain paperwork, database and case notes accurately.
To promote the program within the community and to other service providers and attend organisational meetings.
Oversee service agreement compliance and partnerships with Support at Home service organisations.
To work on projects that enhance sustainability of CCA services
Serve as a member of the Clinical Governance Committee, contributing to clinical oversight and quality improvement.
