**Job Description:**
We are seeking a suitably experienced social worker to join our friendly team at Kalgoorlie Health Campus.
The position provides Social Work services within cancer services at an advanced practice level, and contributes to planning, coordinating, monitoring and evaluating the delivery of timely and culturally appropriate Social Work services for patients.
This role also involves providing expert social worker service in liaison with other members of the regional renal support team.
Key Responsibilities:
* Deliver timely and culturally appropriate Social Work services to patients in cancer services
* Contribute to planning, coordinating, monitoring and evaluating the delivery of Social Work services
* Provide expert social worker service in liaison with other members of the regional renal support team
* Work collaboratively with multidisciplinary teams to achieve high-quality patient outcomes
* Develop and maintain effective relationships with patients, families, carers, and healthcare providers
Selection Criteria:
* Tertiary qualifications in Social Work and eligibility for full membership of the Australian Association of Social Workers or the Society of Professional Social Workers
* Demonstrated experience and advanced level of skills and knowledge within specified area of clinical practice (cancer and/or renal services)
* High level interpersonal, verbal and written communication skills (including information technology, conflict resolution and negotiation)
* Well-developed organisation and time management skills
* Experience working in a cross-cultural setting considering the social determinants, particularly as they relate to Aboriginal Health
* Application of quality and risk management principles with service delivery and practice
* Current C or C-A class driver's licence
