At Nightingale Home Care (NHC), we are on the lookout for a qualified and experienced Community Worker to join our expanding team. We provide quality in-home and community access support to NDIS and Aged Care participants across Geelong, the Surf Coast, and the Bellarine Peninsula.

At this time, NHC is seeking a Community Worker who is an 'all-rounder' and is passionate for providing support to clients with in-home personal care and support with accessing the community.

* Online Induction Modules and Employee Handbook: An informative introduction to our policies, procedures, and values.
* Shadow Shifts: Hands-on experience by working alongside experienced staff to gain practical insights.
* In-House Competencies: Specialised training in areas such as Medication Management, PEG Management, and Manual Handling, tailored to our policies and procedures.

Key responsibilities:

* Provide support to clients in their homes and in various community settings
* Assist clients with activities of daily living in accordance with individual care plans
* Support community participation and social engagement
* Monitor client wellbeing and report concerns to management
* Maintain accurate client documentation and progress notes
* Liaise with families, carers and multidisciplinary teams where necessary
