What you'll be doing
The Clinical Governance Unit (CGU) has the roles of support, performance and conformance in developing and monitoring policies and procedures for improving systems of care. The CGU will oversee the risk management of patient safety and clinical quality by building upon existing incident management and investigation systems.
The main functions of the Medication Safety Pharmacist overseeing medication incident management across SCHN; they will be responsible for monitoring; reviewing, analysing and reporting on all medication safety incidents to identify issues and trends and initiate appropriate actions.
The purpose of position is to:
• Work collaboratively with senior and line managers to provide advice and support in relation to preparing, maintaining, and improving information and reporting systems associated with medication safety and clinical quality.
• Build relationships and work collaboratively with the Network director, department heads and local pharmacy leads.
• Manage and coordinate the Incident Management System (ims+) including data entry, providing data reports to inform decision making; coordination of Reportable Incident Briefs (RIBS) and incident investigations related to medication and patient safety
• Undertake and coordinate quality and safety activities within the facility as directed by the Network Manager of Patient Safety
• Conduct chart reviews and observations to collect data and review practice across the SCHN.
• To work collaboratively with staff across the Network

* Pharmacist with AHPRA registration with no restrictions and at least 3 years of post-registration hospital experience.
* Demonstrated a sound understanding and application of the principles of clinical governance, NSW Health patient safety and medication safety policies and programs and the NSQHS Standards, in association to medication safety standard.
* Demonstrated ability to conduct and facilitate chart reviews, practice reviews, literature searches and audits and apply these to medication safety and quality improvement activities.
* Demonstrated high level communication skills in interacting effectively with clinical staff, senior managers and external stakeholders, including effective written communication skills in the development of reports and briefs.
* Demonstrated excellent interpersonal skills and demonstrated capacity to act as a professional adviser to senior clinicians and managers and proven capacity to foster shared objectives and manage multidisciplinary groups.
* Ability to work independently with minimal supervision and demonstrated time management skills and capacity to successfully deliver outcomes.
* Demonstrated skills and experience in dealing with large volumes of complex information in a timely and efficient manner including demonstrated advanced computer skills in Microsoft applications.
* Able to demonstrate qualities such as integrity, warmth, friendliness and enthusiasm.
