Care Coordinators play a vital role in supporting aged care services.
They manage referrals, onboard new clients, and adapt care plans to meet changing needs. This includes coordinating home visits and working closely with clients and their families to deliver person-centred support.
You will help to improve day-to-day operations by refining systems and ensuring high-quality service delivery. Your work will ensure clients feel supported and valued while contributing to the ongoing growth of our aged care programs.
Key responsibilities include undertaking comprehensive client assessments, monitoring and reviewing care plans, collaborating effectively with internal stakeholders, and developing strategies to address identified needs of Aboriginal and Torres Strait Islander clients.
* Proficiency in utilising technology, including mobile devices and computers
* Strong communication skills and confidence in delivering training
* A current unrestricted Australian driver's licence, reliable vehicle, and willingness to travel
