About the role
The Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) will enhance access to healthcare for "Older Persons'" by addressing service gaps within dynamic Models of Care (MoC).
Improving the health of older persons by providing timely access to specialist aged healthcare, preventing deterioration, reducing hospital presentations and admissions, while collaborating with other healthcare team members.
The primary metaspecialty is 'Aged Care', with the supported development of your Scope of Practice (ScOP) into 'Emergency and Acute Care', 'Mental Health' and 'Chronic and Complex Care'. Departmental locations within the expanding MoC may include but not limited to: Inpatient Units, Outpatient Services, In-reach, Outreach, Community and Emergency.
The Transitional Nurse Practitioner is appointed for a provisional period of up to 3 years to work towards endorsement as a Nurse Practitioner.
