Job Description
We are seeking an experienced and skilled Clinical Care Coordinator & Nurse Mentor to join our team. The successful candidate will be responsible for providing clinical expertise and leadership to the care team, ensuring that all residents receive the highest quality of care.
The role involves managing and resourcing the care needs within the facility, documenting and assessing resident information, and submitting AN-ACC requirements. The Clinical Care Coordinator & Nurse Mentor will also be responsible for coordinating assessment, care planning, and review processes, as well as supporting the admission of new residents.
The ideal candidate will have a strong understanding of aged care clinical assessments, care plans, and accreditation compliance. They will also possess excellent communication and interpersonal skills, with the ability to manage multiple stakeholders and lead clinical outcomes.
Key Responsibilities:
* Provision of clinical expertise and leadership to the care team
* Management and resourcing of care needs within the facility
* Documentation and assessment of resident information
* Submission of AN-ACC requirements
* Coordination of assessment, care planning, and review processes
* Supporting the admission of new residents
* Ensuring accurate information is recorded and in accordance with organisational and legislative requirements
* Input and oversight into facility team rosters and ensuring optimal skill mix available for delivery of care to residents

Required Skills and Qualifications
To be successful in this role, you will need:
* A minimum 1 year experience as a Clinical Care Coordinator or similar leadership position
* A current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)
* Post graduate qualifications in aged care would be advantageous but not essential
* Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC and accreditation compliance and documentation
