An exciting opportunity has arisen for a Lead Practitioner to join the Specialist Older Adults' Mental Health Service (based at Brighton General Hospital).

Main duties of the job
We are looking for a practitioner with a background in Nursing, Occupational Therapy, or Social Work, with experience of working with older people with dementia and other mental health conditions.
As part of the role you will be offering your professional input into the multi disciplinary team, by carrying out initial assessments as well as holding a caseload.
As part of the team structure you will receive professional supervision and be able to access regular CPD sessions and training.
The successful applicant will develop their skills in carrying out assessments, formulating plans and managing a diverse caseload of service users, while maintaining high standards of documentation.
The service is looking to appoint somebody who is highly motivated, enthusiastic, creative and resourceful, and who has a passion for working with older people and for developing good working relationships with carers.
You will have the opportunity to forge close links with the third (voluntary) sector, as well as with other specialist teams within the Trust.

The service operates Monday to Friday, from 9am to 5pm.
The role will involve working from the team base, and across Brighton & Hove as required, with some flexibility around home working, dependent on the needs of the service.

Person Specification
Qualifications
Registered Mental health Nurse Occupational Therapist or Social worker with current registration
Completion of Mentorship Course
Evidence of continuous professional development
Knowledge/Experience
Significant post qualifying experience in the relevant care group
Experience of working alongside people with complex health problems in the designated care group
Experience of supervising staff
Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
Experience of working consultatively with professionals
Experience of conducting clinical assessments including risk assessments
Experience of partnership working within and across various statutory and non statutory teams and agencies
Experience of working within the CPA process
Understanding of clinical governance
Previous experience of working in the community
