Support Coordinator Role
The role of a Support Coordinator involves providing coordination and support services to clients with disabilities, assisting them in achieving their goals outlined in their National Disability Insurance Scheme (NDIS) funded plans.
* Key Responsibilities:
o Collaborate with clients, families, and carers to identify opportunities for skills development, education, and employment.
o Develop and implement strategies to support clients in achieving their NDIS plan goals.
o Maintain accurate case management notes and reports on client progress.
Skills and Qualifications:
* Degree in a relevant field such as social work, psychology, or disability studies.
