Job Overview:
* Support clinicians by delivering delegated psychosocial interventions and contributing to patient recovery.
* Assist with screening, implementing individual and group programs, and providing practical support for independence and well-being.
Key Responsibilities:
1. Conduct clinical screening tasks for patients with complex needs under clinician supervision.
2. Implement therapeutic programs and provide support across transport, advocacy, crisis response, daily living skills, and community access.
3. Deliver education on defined health and living skills topics to patients.
4. Maintain accurate documentation, data entry, and records related to patient care.
