Job Description:
As a Case Manager, you will be responsible for providing case management and community support to parents/carers based on parenting and life skills. You will work in partnership with an Aboriginal Family Support Worker to provide interventions that aim to improve outcomes for children and focus on parents/carers.
Responsibilities:
* Providing case management and community support
* Working with families to identify needs and develop plans
* Facilitating case conferences and meetings
Requirements:
To be considered for this position, you will require:
* A degree in Social Work or a related field
* Eligibility for membership of the Australian Association of Social Workers
* Demonstrated experience in providing parenting support and case management
* A current NT Drivers Licence
